had mine made at a place in Gonzales. They were on Airline on the right just as you get in to town from Baton Rouge. The quality was perfect and the price was under $200. Babins auto glass in Gonzales did mine They are located off of airline Hwy.

to give you an idea of cost at Lawrenece's , my windshield is a bit bigger than the one pictured and it was $160 even , it came with the hardware but I installed it

If you have a little skill you can make your on. First mark the size you need ,then go to a glass shop or a lumber or a hardware store and get the size of plexiglass you need. Then all you need is a piece of plywood and a piece of 2 by 4 and a hair dryer mark the size you need lay it along your 2 by 4 and heat slow and bend it. Then drill your hole using a small drill bit first then the one you need.
